Neurorehabilitation Program
Our center’s Neurorehabilitation Program provides care to individuals with acquired brain injuries, neuropsychiatric disorders and other neurological/behavioral disorders.
We have one unit completely dedicated to neurorehabilitation.
Our programs are based on a motivational system that provides positive reinforcement to residents who attain rehabilitative and treatment goals. Individualized care plans are designed with the objective of discharging to the community or a traditional long-term care facility.
Our program also includes:
- development of skills for community re-entry
- vocational and pre-vocational opportunities
- athletic events such as swimming
The mission of our Neurorehabilitation Program is to provide quality behavioral rehabilitative services that result in the best possible outcomes to individuals who experience or acquire brain injuries, neuropsychiatric disorders and other neurological and behavior disorders.
The program is based on a motivational system that provides positive reinforcement to patients who attain rehabilitative and treatment goals. Individualized care plans are designed with the objective of discharging to the community or a traditional long-term care facility.
Our staff has the expertise to work with extremely complex and difficult clinical situations and is well regarded by referring and consulting clinicians for excellent results and a willingness to place their own family members in our care. We provide treatment to our patients that allow them to get back a measure of function, comfort, and dignity that would not be possible without this special care and approach.
Within each patient’s care plan and program, goals and objectives will be developed to accommodate each individual’s strengths and challenges for rehabilitation. Special priority will be given to developing the skills needed to prepare patients for the demands of their discharged settings.
Prior to admission, a variety of information will be collected regarding the history and status of the patient, as well as any available information regarding past residential placements and projected discharge site will be collected. This information will be forwarded to the interdisciplinary treatment team.
Once admitted, the patient’s placement within the program, a discharge track program will be determined by interdisciplinary treatment team and other relevant stakeholders, including, patient, family, referral sources and clinical staff.
Factors to consider in track program placement decisions may include:
- previous placements
- behavioral obstacles
- learning and acquisition deficits
- patient’s desired discharge site
- current skills deficits
- availability of projected discharge sites
- medical and/or physical limitations
